1) Simply click on the Sign Up button then choose Patient Registration. 2) Please fill in all the required (*) data then click on Save to complete your registration. All the data filled in the registration form will be saved as your personal profile. 3) Please choose a valid email and a valid phone number as they will be the way of contact with TMI or your consultant . 4) An e-mail will be sent to your given e-mail address confirming you as a registered member. 5) You can always go back and edit the data in your personal profile or change your password.

Personal Health Record |
|
Your Personal Health Record (PHR) is simple, freely available and allows you to digitally store all your medical information in one place. It is a summary of your health and medical history. You will be able to access your files at any time from anywhere around the world as long as you have an internet connection. You can securely add or change information at any time.
You have the choice of allowing a consultant to access your PHR which will help him have a comprehensive understanding of your medical problems and to provide you with a proper treatment plan.
Please take some time to complete your PHR by answering as much as possible of the questions in the PHR questionnaire. Please skip or ignore any of the questions that you may find hard or not applicable.
To save all the data don't forget to click the save button at the end of the questionnaire. |

Upload and Archive X-rays, Images, and Test Reports |

Consult Live makes enables you to archive and review your X-rays, images
and test reports so that they will be available for you or your consultant from
anywhere in the world at any time. Scan or digitize your images and then upload
them to your PHR.
- Upload X-rays, images and test reports by clicking on the
button, which allows you to browse your computer for the needed folders and to upload the selected file. - The uploaded files will always be available for you.
- The uploaded files will be available for your consultant(s) only if you authorize him for that.

Search For a Doctorand Schedule an Appointment |
|
1. You can find a consultant and schedule an appointment by clicking on 'Search
for a Consultant and Schedule an Appointment' botton.
|
2. From the console click on 'type of consultation': to choose a 'telephone
consult' or a 'webconference consult'. Remember that for a webconferencing consult you
will need to have PC or notebook available and an internet connection during the time
of the consultation.
|
3. You can search for a consultant by name, or specialty or spoken language, or
simply click on Search to see a list of all available consultants.
|

4. Click on 'View Profile' to review a consultant's CVLabel |

5. To schedule an appointment with your chosen consultant, please click directly on 'Schedule an Appointment'. |

6. The information on your PHR helps your consultant better understand your medical problems and provide a proper treatment plan. On the page of scheduling an appointment, you will have the choice whether to authorize the consultant to view your PHR or not. By clicking on Yes, you authorize the consultant to view your PHR for one week.
By clicking on Yes Until, you can choose the time range, for which the consultant has access to your PHR. By clicking on No, you deprive the consultant of viewing your PHR |

7. To correct time zones, particularly for traveling patients, make sure that
the country you
will be at during the consultation is the same you have
in your Personal Profile. |

8. Make sure that the time and date appearing on your computer's taskbar are correct for your location.
|
9. You can browse and view the consultant's available time slots on the calendar by clicking on:- Day (views the available times on a certain day)
- Week (views the available times during a whole week)
- Month (views the available times during a whole month).

10. From the scheduling calendar you can click "Reserve" on the time and date you prefer.  Appointments that have already been reserved will be shaded in grey and won't be accessible for you.

11. Read the consent and privacy terms, and click on the check box ' I have read and I accept Tele-Med International's consent and privacy terms'

12. Click on Submit. 13. (Payment methods) 14. You now have a confirmed appointment with your consultant. You will receive an e-mail and SMS confirmation.Label

Calender |
|
|
By clicking on Calendar, you can view all your pending consultation times.
|

Test Webcam |
|
|
Don't forget to test your webcam before each consultation by clicking on the Test
Webcam button at the control panel. If you have problems you may need to adjust
the settings (particularly with Windows Vista). Please call us for additional help.
